Home Health Aide

Reports to: Director of Nurses

Department: Clinical

Position Summary:

Works under the supervision of the Director of Nursing or designated registered nurse case manager. Provides direct patient care as assigned by the registered nurse. Provides quality and delivery of home health care services. Assists in the home health care services that reflect the home health care agency philosophy and standards of home health nursing care of assigned clients.

Position Qualifications:

· Home Health Aide is expected to pass competency examination with at least an 80% or better.

· Certified Home Health Aide/or CNA.

· High School Diploma / read, write, and understand the English language.

· Reliable transportation.

· Evidence of sympathetic attitude toward care of the elderly and chronically ill.

· Demonstrated ability to read, write, and carry out directions.

· Evidence of maturity and ability to deal effectively with job demands.

· Good verbal and written communications skills required.

· Attends twelve (12) hours of Aide oriented in-services per year.

· Participates in professional meetings when directed.

· Must be at least 18 years old.

· Must be able to pass a criminal background check.

· Must be able to pass a urine drug screen.

· Must be able to provide 3 professional references.

· Must be able to provide 3 personal references.

· First Aid Training

· CPR Certifications

· Ability to work with limited supervision.

· Ability to follow written and verbal instructions.

· Practice good personal hygiene.

Duties:

· Must be able to practice empathy and genuine concern for patients and their needs.

· Monitor’s patient condition by observing physical and mental condition, intake and output, and exercise.

· Supports patients by providing housekeeping and laundry services, preparing, and serving meals and snacks.

· Assists patients by providing personal services, such as, bathing, dressing, and grooming.

· Ability to lift, turn, and reposition patients that may be bed bound, in a wheelchair or any other restrictive devise.

· Helps patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene.

· Helps family members care for the patient by teaching appropriate ways to lift, turn, and re-position the patient, advising on nutrition, cleanliness, and housekeeping.

· Ability to follow outlined patient care plan, as ordered by physician.

· Records patient information by making entries in the patient journal; notifying nursing supervisor of changing or unusual conditions.

· Maintains a safe, secure, and healthy patient environment by following state regulations, standards, and procedures; maintaining security precautions; following prescribed dietary requirements and nutrition standards.

· Updates job knowledge by participating in required monthly/annual in-service educational opportunities.

· Protects the home care agency and patients by adhering to professional standards, home care policies and procedures, federal, state, and local requirements.

· Enhances service reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

· Occasionally required to accompany patient to medical appointments.

· Monitor patients (vital signs, temperature, respiration, etc.) and report on their condition.

· Able to provide basic food preparation.

· Able to perform light housekeeping.

Physical Requirements:

· Visual/hearing ability sufficient to comprehend written/verbal communication.

· Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.

· Able to bend and stand an average of 6 hours per day.

· Ability to lift up to 50 – 75 pounds.

· Able to work in a stressful environment.

· Able to assess and communicate with patients, co-workers, and general public.

· Is neat in appearance and practice, with good personal hygiene.

RN Case Manager

Reports to: Clinical Supervisor

Position Summary:

Provides direct care and case management for a team of Agency clients in accordance with the state Nurse Practice Act and Agency policy.

Directs: Assigned team members of RNs, LPNs, Home Health Aides, and Personal Care Assistants.

Qualifications:

Current license as a Registered nurse in the state(s) of practice. Bachelor’s degree preferred.
Minimum of two (2) years of nursing experience. Home care experience preferred.
Ability to supervise and direct members of client care team.
Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
Ability to deal tactfully with clients and the community.
Demonstrated organizational and time management skills.
Knowledge of home health regulatory and reimbursement requirements.
Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair.

Essential Functions/Areas of Accountability:

1. Review’s client referral information and responds to requests and inquires as appropriate.

a. Collaborates with clinical supervisor regarding the eligibility and appropriateness of client for home care services.

b. Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for speech therapy unless other arrangements are made.

c. Determines appropriateness of referral for home care services and responds per Agency policy and standard guidelines.

2. Performs initial and ongoing client assessments based on Agency policy and standards of practice to ensure effective and appropriate home care services.

a. Performs comprehensive assessments including OASIS data collection where indicated.

b. Considers the physical, psychological, and socioeconomic needs of the client, the level of family acceptance, and the ability for client needs to be met in the home.

c. Collaborates with physicians, other health care professionals (therapists, social services, pastoral care, supportive services), clients, and families in developing a comprehensive, coordinated plan for care.

3. Directs, plans, and initiates appropriate action independently and responsively in home care situations.

a. Identifies and assesses overt and subtle signs of impending physiologic and psychological changes. Notifies the physician in accordance’s with Agency policies and procedures.

b. Make decisions and/or recommendations reflecting consideration of immediate and long-range effects (frequency of visits, components of care plan, additional services).

c. Confers with the clinical supervisor on a regular basis. Utilizes agency and community resources appropriately.

d. Monitor’s documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.

e. Provides on-call support for clients per schedule.

f. Communicates with other disciplines/departments when required.

4. Manages/supervises a team of RNs, LPNs, Home Health Aides, and Personal Care Aides to provide effective and quality home care services.

a. Identifies the educational needs of team members. Provides training or refers to clinical supervisor to arrange specific training program. Provides or recommends educational resources.

b. Performs or delegates in-home supervision of team members.

c. Leads regular team meetings.

d. Monitors the productivity of team members on a regular basis.

e. Identifies staffing needs and communicates this information.

5. Participates in human resource management to achieve quality service delivery and positive employee relations.

a. Conducts timely performance evaluations consistent with Agency policy. Conducts in-home supervision of staff per Agency policy, regulatory requirements, and as needed.

b. Applies disciplinary procedures in a fair and consistent manner when indicated. Documentation is complete per policy and legal guidelines.

c. Collaborates with supervisors to identify staffing needs and evaluate qualifications and competencies of current and new employees.

6. Promotes personal safety and a safe environment for clients and coworkers.

a. Demonstrates knowledge of safety infection control practices by compliance with policies and procedures.

b. Recognizes and responds appropriately to potentially unsafe situations.

c. Demonstrates safe and competent practice in the use of equipment.

d. Assesses safety of environment and takes initiative to prevent accidents and promote safety.

e. Participates in all mandatory education per established guidelines.

7. Performs job duties in accordance with Agency policies, procedures, and professional and community standards.

a. Maintains confidentiality in all aspects of the job. Does not reveal information from client records to others, except as identified in Agency policy.

b. Secures written confidential documents in a manner that prevents unauthorized release.

c. Participates in the development, implementation, and evaluation of the Quality Improvement Program and activities.

d. Provides care in accordance with state, federal, CHAP, and Joint Commission regulations.

e. Follows established Agency guidelines for protection of confidentiality of all written and computer files.

f. Protects personal password in accessing client and/or personnel files.
